Youth Program Payment Form

    1. Select your home gym (Littleton)

    2. Enter the name of the program participant.

    3. If you are updating billing for multiple participants, select “include additional family member” and add the names of all participants.

    4. Fill out the Contact Information section.

    5. Under Change Request select, “Update payment information on account” and follow the on screen prompt.

    6. Submit Request

  • (this will not cancel your recurring monthly membership and it will still be billed each month)

    Please note: To officially withdraw from a program that is billed on a monthly basis, written notice must be submitted using the form below no later than the 15th of the month prior to withdrawal. 

    1. Select your home gym (Littleton)

    2. Enter the name of the program participant.

    3. If you are canceling for multiple participants, select “include additional family member”.

    4. Fill out the Contact Information section.

    5. Under Change Request select, “Other request”.

    6. Under Additional Comments write that you are ONLY canceling monthly team dues.

    7. Submit Request here

    6. Under Additional Comments write that you are also canceling monthly team dues.
